• When leaders create a culture in which error reporting is rewarded, not punished, errors are more likely to come to the surface and be corrected
• This is acheived at our laboratories by analysing  KPIs (Key Performance Indicators) on weekly basis and statstical analysis on montly basis to determine areas of improvement
